Reflection Paper on Nursing Decision Making


Over many years the organisation and delivery of nursing care has been forged by differing models of care as a result of economic and social changes (Tiedman & Lookinland 7003). Recently, the Garling Report (7008) and the NSW Models Of Care program (7006) triggered several model-of-care projects, including ?Take the Lead?, ?WoW?, and ?Essentials of Care?. These represent professional nursing initiatives in devising improved systems of nursing care in response to recent developments in demand on health services by managerial developments, consumer needs, and the economics of health-care.
Duffield et al (7051) have shown, however, that nursing unit managers, faced with decisions regarding the organisation of care in their particular work setting, tend to base their decisions on pragmatic factors, such as skill-mix available, rather than any pre-conceived model of care.
Chiarella & McInnes (7051) point out that new styles of managerial control of large acute care hospitals have reduced the effective managerial scope of nurses, with consequences for the kind of care nurses can provide to patients.
What emerges is a situation of tension between perspectives shaped by nursing?s professional insight into optimal care for patients (reflected in the models of care), and managerial and resource factors which are real but not necessarily responsive to patient needs.
The Assignment
The Reflection Assignment asks you to draw on things you have engaged with in 55017 to develop an informed reflection on how consumer needs, skill mix issues, and demands on nursing decision-makers might affect the implementation of professionally devised models of care.
The task is this:
? Choose one of the models of care (MOC) that you have been introduced to this semester: information about these can be found at the NaMO website (Dept of Health).
? Using the reading provided, and the website information, consider the implications for your chosen MOC of the following things:
o The importance of a professional commitment by nurses to develop models of care which express nursing insight into optimal care for patients
o The actual and potential consequences for your chosen MOC of:
? Changes in the management arrangements of acute care hospitals
? The increasingly ?pragmatic? styles of decision-making forced on unit managers faced with resource shortage problems
o The need to ensure that models of care reflect appropriate legal and ethical standards of care.
? Prepare a written reflection on your understanding of these implications for the MOC, and
? Close your reflection by addressing the following question:
o Given your evaluation of the impact of the issues listed on your chosen MOC, would implementation of this MOC allow proper expression of the ANMAC Nursing Competencies, or would it compromise
these Competencies?

a. EXPLANATION: All the Models of Care are, broadly, concerned with patient care. However, each Model has a distinctive emphasis or focus. The distinctive focus and the aspects of the Model essential

3. Discuss the prospects of effective implementation of the Model in light of the various constraints (clinical, legal, and ethical) described and reviewed in the available readings (required and other).
